1. A copy of Form I-94 (front and back),
2. A copy of previous EADs, if the applicant has had any,
3. 2 photos with a white background taken no earlier than 30 days before submission,
4. Other supporting documents showing that the applicant is eligible to obtain the EAD (such as I-797 receipt notices, or other documents showing the alien’s non-immigrant status or I-485 application).
