Organization
Coborn’s Pharmacy is part of a supermarket chain serving small communities throughout the Midwest. Currently, there are 24 locations in Minnesota and one in South Dakota. Coborn’s Pharmacy in Little Falls strives to provide the best service to our patients and has been developing new clinical services over the last few years. Two areas of focus are immunizations and medication therapy management. We are also developing a Wellness Center offering cholesterol testing, blood sugar testing, blood pressure monitoring, and A1C testing. As pharmacy services grow, we expect that we will incorporate dietary services and disease state management under collaborative agreement with local physicians in our repertoire.

Resident’s Educational Experience
The resident will manage the pharmacy’s medication therapy management (MTM) practice as well as immunization program under supervision of the preceptor. He/she will work with the Coborn’s pharmacy team to design and implement MTM services to include residents of assisted living facilities in the area. The resident and preceptor will also work closely with staff at Family Medical Clinic to establish MTM services at the clinic. By the end of the residency year, the resident will see patients at both the pharmacy and the clinic. The resident will learn to precept APPE students and hold health seminars for the Little Falls community. He/she will participate in the development a plan for implementation and marketing of the Wellness Center. Along with these responsibilities, the resident will develop skills needed to be community pharmacist in rural Minnesota through staffing requirements and will learn management and leadership skills through working closely with Coborn’s corporate office.

Site Coordinator
Annie Danielson is a 2009 graduate of the University of Minnesota, College of Pharmacy. She has been working as a clinical/community pharmacist at Coborn’s since graduation. She spent her first two years at the St. Cloud - Cooper location, where she provided diabetes education to patients referred by St Cloud Medical Group and managed the pharmacy’s MTM practice. She also further developed the immunization program by coordinating the pharmacy’s efforts to participate in DHS’s Mark of Excellence (MOE) program for influenza vaccines. In meeting requirements for MOE, Annie developed new documentation forms for adult immunization administration as well as new policies for transport of sharps. Annie is a member of the Minnesota Pharmacist Association and the American Pharmacist Association. She is committed to advancing patient care in the Little Falls community through collaboration with other health care providers and expanding MTM services.
